· 2 proofs of residency (utility bills, must be current within the last 30 days), displaying your 911 physical address. Driver's licenses are NOT acceptable.
· Updated shot records
· New enrollees: birth certificates, shot records, withdrawal forms from previous school, and the last report card showing pass/fail
